Drug Addiction Is a Growing Problem in Punjab Essay

1) Throughout the border state of Punjab, whether in villages or cities, drugs have become a scourge. Opium is prevalent, refined as heroin or other illegal substances. Schoolboys sometimes eat small black balls of opium paste, with tea, before classes. Synthetic drugs are popular among those too poor to afford heroin. 2) The scale of the problem, if impossible to quantify precisely, is undeniably immense and worrisome. India has one of the world’s youngest populations, a factor that is expected to power future economic growth, yet Punjab is already a reminder of the demographic risks of a glut of young people. An overwhelming majority of addicts are between the ages of 15 and 35, according to one study, with many of them unemployed and frustrated by unmet expectations. 3) For the Punjab government, the problem is hardly unknown. Private drug treatment centers, some run by quacks, have proliferated across the state, and treatment wards in government hospitals have seen a surge in patients. Three years ago, a state health official warned in a court affidavit that Punjab risked losing a whole generation to drugs. Roughly 60 percent of all illicit drugs confiscated in India are seized in Punjab. 4) Yet when Punjab held state elections this year, the candidates rarely spoke about drug abuse. In fact, India’s Election Commission said that some political workers were actually giving away drugs to try to buy votes. More than 110 pounds of heroin and hundreds of thousands of bottles of bootleg liquor were seized in raids. During the elections, party workers in some districts distributed coupons that voters could redeem at pharmacies. 5) Punjab’s reluctance to treat the drug situation as a full-blown crisis is partly because the state government itself is dependent on revenue from alcohol sales. Roughly 8,000 government liquor stores operate in Punjab, charging a tax on every bottle — an excise that represents one of the government’s largest sources of revenue. India’s comptroller found that liquor consumption per person in Punjab rose 59 percent between 2005 and 2010. 6) A sociologist in Amritsar, surveyed 600 drug addicts in rural and urban areas of Punjab and found that they were usually young, poor and unemployed. He said that most villages did not have health clinics but did have three or four drugstores, which often made sizable profits selling pills and other synthetic drugs to addicts who cannot afford heroin. 7) Opium has a long history in Punjab, and was commonly and legally consumed here before India and Pakistan gained independence in 1947. Today, Punjab is a primary gateway for opiates smuggled into India from Pakistan and Afghanistan. Opium is also grown legally in India for medicinal purposes, and some of the crop arrives in Punjab on the black market. 8) The problem is prevalent in middle-class enclaves, where some users are hooked on heroin. One impoverished neighborhood of Amritsar, called Maqboolpura, is known as the Village of Widows — because so many young men have died of drug abuse. 9) In Kazikot Village, about a two-hour drive from Amritsar, a local nongovernmental organization tries to prevent the spread of H.I.V. by regularly distributing clean syringes to addicts. The group’s workers say there are 48 hard-core addicts in the village (out of more than 2,000 people) but that many other people use drugs. Government officials have sponsored â€Å"camps† here, with health officials providing antidrug information or trying to persuade addicts to undergo treatment — neither of which, villagers say, has been successful.

A hero must be made through heroic action and these characters completed heroic action while standing up for their own authority, which were the lords of the sky and the lords of the castle. While on their voyages they each encountered sin as well. The author writes as a liberal and as a feminist in putting forth a critique of the ways in which multiculturalism – as it was, and still is, being advocated by various activists and academics – poses real dangers to the rights and interests of girls and women that belong to minority groups. The way she does this is admirable: In a manner that is clear and with not-so-well-known examples, she shows how most of the major cultures of the world puts a premium on â€Å"maintaining control of women† and how the advancement of the rights and interests of women conflict and clash with particular practices within the cultures of minority groups. She is admirable in standing firm with the feminist insight that â€Å"more often than not, sex discrimination is far less overt. In many cultures, strict control of women is enforced in the private sphere by the authority of either actual or symbolic fathers, often acting through, or with the complicity of, the older women of the culture. † This, I think, is her central argument in making the case that there are â€Å"tensions,† if not contradictions, between feminism and multiculturalism. Despite my respect for Okin’s arguments and mode of argumentation, I also have a few critical remarks: (1) For a couple of times in the essay, Okin referred to â€Å"more patriarchal minority cultures exist[ing] in the context of less patriarchal majority cultures. † This, I think, is one of the great assumptions of her essay – the situation that is implied to be the context of her thinking and writing. The question therefore arises: Is she talking about North America and Europe? I wish that Okin made this assumption an argument, changed it from being implicit to being explicit. This would have made her case stronger, or at least clearer, and not prone to charges of Eurocentrism in this context, or the assumption that states in North America and Europe are liberal ones that are good or even better for girls and women to grow up in. Not that I disagree with this belief completely, but I believe that this must be shown rather than merely assumed. Had Okin made this explicit, perhaps she would have posed and addressed related issues such as the degree of â€Å"liberalism† or â€Å"feminism† of these states with regard to girls and women, especially when compared with cultures of â€Å"minority groups. † (2) I also hoped that Okin presented what advocates of â€Å"group rights† are fighting for, or what they themselves say they are fighting for. I think that she will agree that these advocates are not fighting for group rights for the sole purpose of controlling or oppressing women. By not presenting the objectives of these advocates, however, we are left with the impression – one-sided, surely – that the enforcement of these group rights by the state will only lead to the tighter control and greater oppression of women. Had she presented the objectives of these advocates, we readers will probably be given the chance to weigh these against the dangers that she had posed. It is not that the rights of minority groups should take precedence over the rights and interests of girls and women within these groups. But the fact that these objectives can constitute real gains for populations of minority groups, and not just attacks on the rights and interests of girls and women suggests that the struggle for the rights and interests of women on the one hand, and of the minority group on the other, could be seen as complementing each other, part of a struggle to expand freedoms of various peoples. (3) I wish to make a few comments regarding various points of Okin’s paper. (a) Okin defends feminists (needless to say, from one culture) who criticize anti-women practices in other cultures. I agree with her on this score. Such a view, however, should be balanced by the view that real change happens only when its constituents fight for it. I agree that we can pass judgment on the tyranny of Saddam Husein but I do not agree that this gives us license to attack Iraq. Real change in Iraq should come from its people, not from other countries. (b) I have the feeling that control and oppression of women in the family sphere is too systemic a problem for it to used as a yardstick for measuring gains that group rights can actually bring a minority population. (c) One more thing that’s sorely missing in Okin’s essay is an appreciation of how changes in history take place. I think that piecemeal reforms for oppressed people are important and can lead to bigger things. What she does, from one perspective, is to pit piecemeal reforms against systemic changes – which is not, I think, how changes in history take place.

The Uncertainty Reduction Theory states that people tend to gather information about people to reduce uncertainty about them. This uncertainty is common not only when meeting other people for the first time, but it is present in all types of new situations. It is essential to our survival to constantly assess the environment and threats, whether they are real or perceived. Uncertainty Reduction Theory gives us a guideline on how and why we react to new situations. The Uncertainty Reduction Theory can be linked to our evolutionary past in which we had to assess the threat level of potential predators and detect their presence. â€Å"Heider’s notion that man seeks to â€Å"make sense â€Å"out of events he perceives in his environment,† is reminiscent of Neil deGrasse Tyson explaining why human brains tend to create patterns (Craig and Muller, 2007, p. 327). The reasoning was that our ancestors who were constantly under threat of predators would sometimes face situations in which there may or may not be a predator in that bush or high grass. If you look up at a tree, your brain naturally creates patterns among the leaves and branches where you may visualize some type of shape that may look like a lion, a person’s face, or a triangle. In the physical reality, there is no lion, that’s just the way the tree is shaped, or there actually is a lion there. Those that created patterns and visualized the lion whether a preda tor was there or not, ran away and survived. Those that didn’t create the patterns would have either survived(if there was nothing there) or have been killed(if there actually was a lion). In a type of Pascal’s wager, those that evolved brains to create patterns and in turn, took no chances on their livelihood survived. This same evolutionary pressure to â€Å"make sense† or create patterns of environmental stimuli to assess threat level can be adapted to interpersonal communication among individuals of a species. Humans can communicate verbally through language and that would be the ideal way to detect the threat level of an individual or group after assessing body language. Finding certainty in the uncertainty of whether there’s a lion in the bushes or whether a person is a potential friend or foe is essential to survival and interpersonal communicative behaviors is the medium in which uncertainty can be converted to certainty. Survival relies on awareness and our consistent surveying ability. It is optimal behavior to gather as much information as possible when new people or situations arise. Gathering information is crucial to making ideal decisions. Decision making becomes easier when you have as much information as possible.

He debates that in order to be a tragic hero the hero doesn’t need to be a king or have any noble background and instead, the common man can be considered a tragic hero as well. Arthur Miller justifies that the tragic flaw is when a tragic hero refuses to accept anything that may affect their personal dignity. Arthur Miller’s ideas on tragedy are precise and can be applied to a various number of tragedies seen throughout media. In the movie â€Å"A Time to Kill† Jake Brigance one of the main characters is a lawyer who decides to defend his African American friend Carl Lee after he is charged for killing two Caucasian men who previously raped his eleven year old daughter. The setting of the movie is in a very racist community therefore when Jake decides to defend Carl the tragedy begins. In the movie Jake encounters many obstacles one of the first of them being when his wife and child are forced to leave because their safety is not guaranteed in their community anymore. Brigance can simply drop the case and have his family stay with him however he decides to stick to what he believes in is right and allows his family to leave. Throughout the movie you can see that Jake is becoming very lonely not having his family by his side but he pushes those feelings. At one point in the movie Jake loses his house due to a racist colt also known as the â€Å"K. K. K†. While Jake is at work the colt decides to burn down his entire house unaware that his only family he has left which is his Dog is in there. Once Jake comes home to see that his house is severely damaged he slips into a state of denial and refuses to believe that his only companion he has left may have died. In the movie scene you can see that Jake is struggling so much he begins to doubt himself. â€Å"Were going to lose this case, Carl lee†¦ I want to cop a plea, maybe Buckley will cop us a second degree murder and we can get you just life in prison. Jake could have decided not to defend Carl Lee and to allow any other lawyer do to the job but he refused to accept the fact that the court could allow an eleven year old girl to be victimized and not have any justice served because of the color of her skin. In the book â€Å"Rome Juliet† Romeo and Juliet experience tragedy because of accident and bad luck, the unfortunate fact that Friar Lawrences letter never gets to Romeo in Mantua. Miller says that in order to be classified as a tragic hero you must be willing to lay down your life in order to keep your dignity and Juliet does this. O happy dagger! This is thy sheath; there rust and let me die! (Juliet). When Juliet does not receive the letter form Friar Lawrence and thinks that Romeo is dead she realizes that in her battle to get what she wants she has lost and therefore lays down her life for Romeo. This shows that Juliet a child who wasn’t royalty suffered just as much as someone with a noble background would suffer if they were going through the same issue. I can speak about tragedy and being a common man because at one point in my life I’ve experienced it. Last year in grade ten I was diagnosed with a tumor on my thyroid. When the doctors told me I had a tumor on my thyroid they explained to me that they were unsure if it was cancerous or not. For two weeks I had to wait for the results during those weeks I had this fear following me around and all I could think about was whether I had cancer or not. I went through several tests. After all my results from the test came back the doctor explained to me that my tumor was benign which meant it was not cancerous but the problem was it wouldn’t stop growing. It grew to the point it seemed like I had an Adam’s apple. The only choice I had was to cut out the tumor or take pills which could control it however I would have to take the pills throughout my entire life, the pills were expensive. I decided to get the surgery done instead and I now have this very large scar on my neck. As you can see I also experienced tragedy when I was diagnosed with the benign tumor. Common men can be tragic heroes as well regardless of the reputation or the status they hold. Tragedies are not meant for just people with high positions or everyday people who would read or watch media that had to do with tragedy wouldn’t be able to comprehend it. Everyone suffers some type of tragedy in their life which allows us to understand tragedies much better.

Conflict Resolution and Communication in Healthcare Reflective Writing on Critical Incident in the Clinical Experience with Integration of Leadership Theories in Analysis. This write up aims at exploring a conflict that occurred in an urban private hospital concerning a patient, relatives and medical team. Its purpose is to reflect and critically study and understand a clinical incident and use it as a learning episode by use of reflective model. To identify the measures put in place to manage the conflict and the type of leadership skills and leadership theories used to resolve the conflict. Gibbs reflective cycle will be used to expound on the scenario as it unfolded. This is because Gibbs is clear and thus allows description, analysis and evaluation of the experience helping me to make sense of experiences and examines my practice (Gibbs, 1988). Conflict can be defined as disagreement in which the people involved feels that their needs, interests and concerns are threatened. Health care related conflicts are complicated because the care process is ongoing and involving a lot of emotional effort and professional relationships therefore tends to occur frequently and thus interpersonal conflicts, (Johansen, 2012) Incident description I once worked in private urban hospital and while there on my practice I encountered a scenario that left me thinking on how well our patients are informed and involved in their care. On this particular day I reported to work as usual and received the report in a high dependency unit, about the unit and the patients admitted. Special report given was that there were some pending investigative procedures on two patients to be facilitated by one consultant. This investigation were requested on the previous day therefore follow up was to be done during this shift to ensure they are done. After receiving the report we went for quick unit round and assessed the patient status and then the unit manager embarked on contacting the consultant to come and do the tests, efforts to trace him were futile since his phone was not going through. The unit manager contacted the second on call for the same and said that he was out of town thus not in a position of performing the test the same day. In this particular organization the patient and relatives are appraised on daily basis of everything that it’s to be done therefore the relatives were eagerly waiting for the test result when they came back during the visiting. When the relatives came back they wanted to know the test results unfortunately they were not available because the test had not been done, the manager and the doctor covering the unit tried to explain to them in vain. They became very furious and agitated and started using abusive language. Efforts to make them understand were futile. They walked out of the unit to have a discussion among themselves and in 30minutes they were back demanding that their patient be discharged against medical advice so that they can outsource the service elsewhere saying â€Å"you people do not care about our patient but we care about him and we are ready do anything possible to facilitate this test been done†. They signed the form for leaving against medical advice and left. From this episode a crisis meeting was convened to include the medical director, director of nursing the unit in-charge and the doctor who was covering the unit then and a critical analysis and evaluation of the scenario was done and it was resolved that this particular consultant always have an issue with his patient whenever he is on call and therefore it was agreed that the director of medical services and nursing service will meet with the doctor and have a one on one discussion in order to prevent a recurrence. It was also agreed that we follow up on this particular patient and find out what was their progress and whether they went for the test and to even apologize to them and let them understand that we care and patients interest come first and only that there was a problem on that day, the patient was traced and found to be doing well and they never went for the test on that particular day they had to wait until the next since they could not get the service where they went because it’s the same consultant who doing the procedure in that facility and therefore opted to come back to our facility for readmission and they very apologetic and remorseful for the actions â€Å"we regret our action and we want to apologize for unnecessary attention that we demanded for while your efforts was to ensure for better outcome of patient. We are sincerely sorry for the disturbance and our shameful acts† we reassured the relatives and readmitted the patient for continued care. Feelings This experience made me feel like we failed no matter how we tried explaining to the client and his relatives it never seemed to be working. I also felt that we failed to meet our goal of ensuring that patients are satisfied with the services they receive, though the unit manager was very composed throughout this situation and handled the relatives with a lot of care and caution she never seemed to be worked up by their demand. This leader portrayed very good qualities that I really admired and felt that I should emulate, she had a good charisma. Felt that she was a transformational leader because she displayed charisma and we as her juniors followed her way of instituting measures in place. She was able to reason with the relatives and explain on the events as they were unfolding even though they never paid any attention or reasoned with her, she explained of the challenge we were facing in the situation. She engaged the relative by stimulating them to understand through use of logical questions and exploring on their assumptions (Hendel, Fish Galon, 2005). I also felt the manager had some qualities of transactional leader because she was able to monitor the performance and step in to correct the situations as they arose and also utilized passive management since the problem arose unexpectedly and therefore rushed in to handle the situations (Hendel, Fish Galon, 2005). Utilization of these skills gave the manager an upper hand in handling the furious relatives and creating their understanding even though they decided to leave the hospital. Convening a crisis meeting by the management was a very important step as this created an opportunity to address the issue as it happened and come up with solutions to prevent recurrence and provide steps in handling the situation in case it recurs. The affected consultant was also give an opportunity to explain what transpired and he had a genuine reason because he was in theater doing a procedure and he called back to the facility after he was done to inquire why we were calling him them. I felt that the relatives also have role to play during care of their loved by been supportive and establishing a trusting professional relationship with the health professional to establish a common working ground. Decision to withdraw the patient from the hospital was solely made by the relative and the patient felt vulnerable and tossed around since he could not make sound decision there compromising on the patient care in the name of seeking for a service. Evaluation This episode provided a learning situation for me as I experience how difficult it might be in handling clients that you have already established a professional relationship with, and with all efforts to try and let the clients understand the situation by providing them with information. What was good about this incident is that the manager was very calm and composed all through and understood the feelings of the relatives concerning their loved one. She was able to utilize her skills optimally to handle the situation. What was bad about the situation is that the investigation was not carried out and the clients felt ignored or left out and thus opted to leave the hospital and seek services elsewhere as fate would have its way they never accessed the services on the material day because most of them are outsourced and the same consultant does the procedure in the facility they went too. They were embarrassed by their behavior and even come back to apologize on the same and we reassur ed them. Eventually they brought the patient for readmission. Study done in an Israel hospital on conflict resolution on nursing managers indicated that they mostly utilized transformational leadership style because it allows for creativity and flexibility in problem solving and scrutinizing all protocols and policies set to manage situation through critical analysis (Hendel, Fish Galon, 2005). Actions to withdraw the patient and interrupt care compromised on the patient health status and even the expected outcome. Analysis From this situation the patient’s relatives were right in demanding for action because they had been explained about the test, its importance and why the result may be needed to make decision on the way forward in patient’s management. On the same case having not done the test on the same day would not have worsened patient’s condition and still medical management would still be ongoing as we awaited the test to be done. The crisis meeting organized ensured the lasting solution is availed in order to improve on patient care and therefore a ensure quality care leading to better patient outcome. To effectively resolve a conflict a better understanding should be ensured through clear communication and understanding the crisis leading to disagreement. Therefore conflict resolution becomes an important aspect in prevention of errors occurring (Sportsman Hamilton, 2007). The unit manager utilized the transformational theory and understood that followers are an important aspect to facilitate leadership and that the follower is always accountable and with skills to critiques scenarios and issues as they unfold just like the leader does. Similar to the incident the manger took control of the situation vividly which was a learning opportunity for in the unit that day as potential leaders (Daft, 2008). Conclusion From this incident it can be concluded that the health team played their part well though an element of ineffective communication is evident since the relatives were aware that the test needed to be done in order to make decision on the management therefore emphasis had been put across thus explains their fury when it was not done. Leadership experience plays an important role in ensuring or equipping the leader with skills to handle situation in future thus creating an understanding of the theories used in leadership. For one to be effective leader one must learn whom they are what are the strengths and weaknesses establish a personal philosophy and be strong to stand firm and represent what we believe in terms of patients care and skills of conflict resolution (Daft, 2008). Action plan To work as a team is an important aspect of nursing and that effective communication is paramount in ensuring that the patient and relatives are well informed on the care provided to their loved ones. This experience has been an eye opener in me and it has made me realize that I can never be ready to solve a conflict this because each occurrence is always unique and present in a different manner, therefore utilization of leaderships skills become important.
